Please only complete this registration form AFTER we have met on a consultation call and you have agreed to start lessons on a specific date! 

Also be aware that this form will ask for your credit card/debit information - that is to set up autopaid invoices. I want it to be clear that autopay does not start charging you until I turn it on!
